Privacy Policy
How we protect your personal and health information
U Surgical Centre, part of the U Vision Group family, is committed to protecting the privacy and confidentiality of your personal and personal health information. This policy explains what we collect, how we use and protect it, and your rights. It reflects our obligations under Ontario’s Personal Health Information Protection Act (PHIPA) and Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).
Information We Collect
Through this website we collect only the limited information you choose to provide — such as your name, email address, phone number, and a brief reason for your inquiry. In the course of providing surgical care, and as directed by your referring specialist, we also hold personal health information necessary for your safe treatment (for example, your procedure details, relevant medical history, and the records generated during your care).
How We Use Your Information
We use your information to coordinate and deliver your surgical care, to communicate with you and your referring specialist, to meet our legal and regulatory obligations, and to respond to inquiries you send us. We do not sell your information, and we do not use it for advertising.
How We Protect Your Information
We maintain administrative, physical, and technical safeguards appropriate to the sensitivity of the information we hold. Wherever practical, your information is stored on systems located in Canada (Canadian data residency). Access is restricted to those who need it to provide or support your care.
Disclosure of Information
We disclose your personal health information to your referring specialist and care team for the purpose of your treatment and follow-up, and otherwise only with your consent or as permitted or required by law.
Records Retention
Surgical and medical records are retained in accordance with the requirements of the College of Physicians and Surgeons of Ontario — generally at least ten years from the date of last entry, or, for a patient who was a minor, ten years from the date the patient reached or would have reached the age of majority.
Third-Party Services
Some functions across the U Vision Group network rely on third-party service providers (for example, appointment scheduling through Calendly). Where a provider processes information outside Canada, that information may be subject to the laws of the jurisdiction in which it is held. We work to limit such transfers to what is necessary and to providers with appropriate safeguards.
Email Communications
In keeping with Canada’s Anti-Spam Legislation (CASL), we send commercial electronic messages only where we have your consent or another lawful basis, we identify ourselves clearly, and we include a way to unsubscribe at any time.

You have the right to access and request correction of your personal health information, and to ask questions about how it is handled. To make a request, please contact us using the details below.
